Notice of Data Security Incident

McKenzie Memorial Hospital d/b/a McKenzie Health System is committed to protecting the security and privacy of the information we maintain. We responded to and addressed a security incident that disrupted the operations of some of our IT systems. We also mailed notification letters to patients whose information may have been involved in a security incident that disrupted the operations of some of our IT systems

McKenzie Health System first identified the incident on March 11, 2022, and immediately took steps to secure our systems, launched an investigation with the assistance of a third-party forensic investigator, and notified law enforcement. The investigation determined that an unauthorized party accessed our systems from March 10, 2022 to March 11, 2022, and removed some files.

On September 22, 2022, we completed our review and analysis of the files involved in the incident. The review determined that the files contained information belonging to McKenzie Health System patients, including names, contact information, demographic information, dates of birth, Social Security numbers, driver’s license numbers, diagnosis and treatment information, prescription information, medical record numbers, provider names, dates of service, and/or health insurance information.

For patients whose information may have been involved, we recommend that they review the statements they receive from their healthcare providers and contact the relevant provider immediately if they see services they did not receive. Additionally, for patients whose Social Security numbers may have been involved, we are offering complimentary credit monitoring and identity protection services through Experian.

We take this incident very seriously and sincerely regret any concern this may cause. To help prevent something like this from happening again, we have implemented additional safeguards and technical security measures to further protect and monitor our systems.
