Sanilac County, Mich. – A new series of free workshops aimed at helping individuals manage chronic health conditions will kick off in May, thanks to a partnership between McKenzie Health System and Michigan State University (MSU) Extension.

The Personal Action Toward Health (PATH) program is a six-week evidence-based series that offers guidance and support for adults living with long-term health conditions. The workshops are designed to improve participants' quality of life by teaching practical skills for managing symptoms, building healthy habits, and communicating more effectively with healthcare providers.

The PATH series will be held on Mondays from May 12 through June 23, 2025, excluding Memorial Day on May 26. Each session runs from 1:30 to 3:30 p.m. and will take place in the back dining room of McKenzie Health System’s Hospital Campus, located at 120 Delaware Street in Sandusky.

The program is open to adults with chronic conditions such as diabetes, arthritis, high blood pressure, or heart disease, as well as their family members or caregivers. Topics covered will include healthy eating and exercise, pain and stress management, medication use, goal setting, and how to evaluate new treatment options.

“This workshop series is a fantastic opportunity for anyone living with a chronic condition to take positive steps toward better health,” said Nina Barnett, Public Relations and Foundation Coordinator at McKenzie Health System. “It’s about giving people the tools and confidence to actively manage their health in daily life.”

Workshops will be led by experienced MSU Extension educators Kris Swartzendruber and Jacqui Rabine. The PATH program is funded by the Association for Community Living and is free to attend.

There will be limited spots available and pre-registration is required. To sign up or learn more, visit www.McKenzieHealth.org/PATH or call 810-648-6127.
