MHS Family Healthcare Providers Earn Patient-Centered Medical Home Designation

Blue Cross Blue Shield of Michigan has designated the following providers as Patient-Centered Medical Home (PCMH) Practices for 2018: Dr. Mark English, Dr. Matthew Gormley, Dr. James L. Sams, and Dr. Michael Merkler.

McKenzie Health System (MHS) providers have received this designation, which recognizes a practice for excellence and efficiency in caring for its patients, for the past 5 years. The current designation period runs from July 1, 2018 through June 30, 2019.

Blue Cross Blue Shield of Michigan joins United Healthcare Community Plan, Priority Health and Meridian Health Plans in recognizing MHS’ commitment to quality patient care through the PCMH Designation.

In order to earn the PCMH designation, providers must meet strict standards for quality, efficiency and safety. PCMH designated practices meet patients’ needs by providing more personalized, whole-person care. This change in practice patterns has resulted in higher quality care, with better outcomes and lower cost.

In practice, the PCMH designated provider offices focus first on engaging the patient in their care while helping patients keep track of their appointments with specialists, tests, and prescription refills in order to assist them in meeting their healthcare goals. The strategy is to detect health problems early; when treatment options and outcomes are the greatest. In addition, at MHS, a Preventative Care Coordinator assists the PCMH certified provider’s office in identifying additional services a patient might need (such as transportation or dietary counseling).

“The patient-centered medical home model is centered on the patient,” explains Billi Jo Hennika, Chief Operations Officer. “All MHS family healthcare providers take a comprehensive approach to care to assist patients in meeting their healthcare goals.”

Throughout its six family healthcare offices, including the new after-hours clinic, MHS’ team of professionals work together to ensure that families receive superior, compassionate healthcare.
