Nation’s largest PCMH program transforming primary care in Michigan
Blue Cross Blue Shield of Michigan has designated the following providers as Patient-Centered Medical Home (PCMH) Practices for 2016: Dr. Mark English, Dr. Matthew Gormley, Dr. James L. Sams, and Dr. Michael Merkler.
McKenzie Health System providers have received this designation, which recognizes a practice for excellence and efficiency in caring for its patients, for several years now. This designation period runs from July 1, 2016 through June 30, 2017.
Blue Cross Blue Shield joins United Healthcare Community Plan, Priority Health and Meridian Health Plans in recognizing McKenzie Health System’s commitment to quality patient care through the PCMH Designation.
“The patient-centered medical home model is centered on the patient,” explains Dr. Merkler. “All McKenzie family healthcare providers take a comprehensive approach to patient care helping patients meet their health goals and keeping track of all their specialist visits, test results and prescriptions.”
In order to earn the PCMH designation, providers must meet strict standards for quality, efficiency and safety.
In practice, the PCMH designated provider offices focus first on engaging the patient in their care while helping patients keep track of their appointments with specialists, tests, and prescription refills in order to assist them in meeting their healthcare goals. The strategy is to detect health problems early; when treatment options and outcomes are the greatest. In addition, at McKenzie Health System, a Preventative Care Coordinator assists the PCMH certified provider’s office in identifying additional services a patient might need (such as transportation or dietary counseling).
Throughout its six family healthcare offices, McKenzie Health System’s team of professionals work together to ensure that families receive superior, compassionate healthcare. Physicians, Nurse Practitioners, and Physician Assistants take on the role of caring for patients of all ages every day, ensuring the legacy that McKenzie Health System is recognized for. “The current trend in healthcare is what is known as population health management,” says MHS President and CEO Steve Barnett. “MHS has invested in the technology that will compile a patient’s data from various providers across the healthcare community into a virtual “dashboard” so physicians have a holistic view of their patient. This is crucial to providing the quality healthcare people have come to expect from their MHS doctors.”
