LPN/RN Community Care Coordinator (ROC)

POSITION POSTING

Title: LPN/RN Community Care Coordinator

Department: Healthcare Practices

Status: ROC – 16 hours a week

Hours: Hours may be adjusted in response to workload demands. May require weekend or evening hours. Low census may be utilized in accordance with hospital policy.

Qualifications/Position Requirements:

1. Graduate of an accredited School of Nursing, required.

2. LPN or RN with required renewals, BSN preferred but not required.

3. Previous experience in Care Coordination, Case Management or Public Health preferred.

4. Proficiency utilizing software programs and has the ability to work in an Electronic Medical Record.

5. Previous management experience in nursing supervision, nursing management or equivalent, preferred.

6. Ability to adapt to change and manage multiple projects simultaneously.

7. Demonstrates the ability to establish good working relationships and effective communication with providers, peers, patients, families and other support personnel.

8. Supports and contributes to the total health system by maintaining a health system-wide perspective and serving on clinic/hospital committees.

9. Influences the direction of Health System performance by maintaining a positive perspective/attitude.

10. Demonstrates the ability to communicate in speech, writing and reading the English language.

11. Demonstrates administrative skills essential to the job.

12. Demonstrates the ability to handle multiple tasks in a stressful situation while maintaining composure and excellence.

13. Demonstrates maturity and emotional stability with evidence of a sense of responsibility, initiative, honesty and positive attitude.

14. Demonstrates the ability to keep detailed records, report results and work independently as needed.

Licensure/Certification

1. Current BLS certification required within 6 months of hire.

2. LPN or RN with required renewals, BSN preferred but not required.

Job summary:

The LPN/RN Community Care Coordinator will collaborate with providers and other care team members to assist patients with chronic care management, prevention/wellness and effective transitions of care. The goal of the LPN/RN Care Coordinator is to identify patients that are at risk for worsening of a chronic condition, those living with existing complex chronic disease, and those utilizing inappropriate care resources due to an unmet need for primary care access. The LPN/RN Care Coordinator works with the care management team to promote effective community resource utilization, patient self-management of chronic disease, and timely access for the right care at the right time for patients. The LPN/RN Care Coordinator collaborates with providers in performing annual wellness visits.
