Diabetes Self-Management Education Service (DSMES)

About the McKenzie Health System DSMES

The McKenzie Diabetes Self-Management Education Service (DSMES) is designed to provide the skills and tools needed to aid in the day-to-day management of diabetes and to maintain and increase life’s physical activities. As a result, people will be better equipped to face daily challenges of living with diabetes.

This service offers 10 hours of education. One hour of individual and 9 hours of group class.

This program requires a referral from your primary care provider. We will take care of the paperwork for you and are happy to check your insurance for verification of payment and any co-payments. All you need to do is commit to a “HEALTHY LIFESTYLE CHANGE”

The McKenzie DSMES is accredited by the American Diabetes Association (ADA) and is run under the guidelines of the Michigan Department of Community Health. DSME includes:

  • A one-hour initial health assessment
  • Three classes, 3-hours each, of group education on the basics of diabetes care
  • Class topics include Healthy Eating, Being Active, Taking Medications, Monitoring Glucose, Chronic Complications, Lifestyle & Healthy Coping, Diabetes Distress & Support
  • Goal Setting & Support
  • Materials are provided to help clients with the behavior change process
  • Education and support staff include a registered dietitian, registered nurse, registered pharmacist, social worker, and physical therapist – all have gone through additional training specific to this service
  • Participants may also be eligible for Medical Nutrition Therapy appointments and follow-up support opportunities to help with continuing lifestyle changes
  • Your education and support continue annually with 2 hours of diabetes education with one of our educators and or 2 hours with our dietitian
  • Dealing with Diabetes is a Lifestyle Change

Availability of Services

The DSMES program is available to patients with a provider referral.

Cost

This service is covered by Medicare, Medicaid and Commercial insurances. We are happy to check regarding coverage.

Class Schedule (please call to confirm schedule)

Classes are 3 hours in length. One class per week for 3 weeks.

Nov. 14, 21, & Dec. 5, 2023. 1 pm - 4 pm

Jan 2, 9, 16, 2024................ 1 pm - 4 pm

March 5, 12, 19, 2024.......... 1 pm - 4 pm

May 7, 21, 28, 2024........... 8 am - 11 am

July 9, 16, 23, 2024............ 8 am - 11 am

Sept 10, 17, 24, 2024......... 8 am - 11 am
